This is a sample of the HICS System binder Tabs for the Medical Care Branch Director. The intent of the binders is to provide a single place for organizing and storing HICS IV information. As the binders are used and updated, the binders become part of the hospital’s cumulative memory and “lessons learned” for responding to an emergency.

Tab 3, Job Action Sheets, Subordinates

Some HICS staff members must distribute other HICS IV Job Action Sheets and Forms to subordinates. This table lists the HICS IV Job Action Sheets and Forms  to be distributed, arranged by HICS staff position. For example, the Medical Care Branch Director would give the Job Action Sheet for the “Inpatient Unit” and relevant HICS IV Forms (pages 2 – 19 in the binder) to an appropriate hospital staff member.

Each binder has it own RESTOCK file on CD in Microsoft Word to replenish documents and forms that are used during a HICS activation.

Tab 3, HICS IV Forms

The HICS System contains duplicates of the HICS IV forms, converted into Microsoft Word files. Binder No. 24, “HICS FORMS,” contains duplicates for all 20 HICS IV forms developed by the California EMSA.

Tab 3, Organization

Tab 4, Message Form Sample

Tab 4 includes an example HICS System Message Form, specially designed to help HICS staff in the Hospital Command Center record information and to follow-up on coordination efforts. This form includes a “tickler Box” at the bottom to help the Operations Section Chief follow progress on critical coordination by key members of the Hospital Command Center.

Tab 4, Operations Room Layout

Tab 4 in each HICS System binder contains a recommended Layout for the Hospital Command Center operations room. The Operations Room is where overall situation awareness is displayed and periodic staff briefings are conducted.

Staff positions are indicated around a large central table in the Operations Room. Dark lines around the walls of the room represent HICS System Walll Displays which are located near the staff who must update them.

Tab 4, Binders to Distribute

During activation of the Hospital Command Center, the Initial Tasks sheet for the Incident Commander refers to this page for other binders to distribute. In turn, each activated binder activates other binders, and so on. Some binders contains Job Action Sheets for subordinate HICS IV jobs. This page represents the primary procedure for “unfolding” the entire HICS System and Hopsital Command Center.

Tab 5, Operational Log

Operational Log forms are stored in Tab 5. The form is a duplicate of the California EMSA form developed for HICS IV. In this Tab, HICS staff members maintain a meaningful log information for replacements or for incident records.

This is the Instruction Page for Tab 6. HICS System users can use the Message Form developed by the California EMSA for HICS IV, or use the supply of triplicate Message Forms designed specifically for the HICS System.

Tab 7, Hospital Resource Directory

Tab 7 is the place to store the Hospital Resource Directory. HICS System users can insert HICS IV Form 258, “Hospital Resource Directory,” or other effective formats for storing contact information about resources available to the hospital during a disaster.

Tab 8, HICS Forms

Tab 8 in each HICS System binder lists the HICS IV Forms that are pertinent to the HICS job. In this case, HICS Form 204 would be included behind this page. The other HICS Forms are found in Tab 4 or Tab 5.
